Clinic Information

The Phoenix High School clinic is staffed by Mrs. Beth Hortman.  Ms. Hortman can be reached by calling 770-277-4495 or by emailing Beth.Hortman@gcpsk12.org.

The Phoenix High School clinic is staffed from 7:30 AM to 5:30 PM on regularly scheduled school days.

The Phoenix High School clinic is located in Room 1.248 (back of the building behind the Gym and adjacent to the Parenting Center).

It is important to have a record of all students who have special medical conditions that may require assistance and/or monitoring during school hours  (i.e. asthma, severe allergies, diabetes, etc.).   Parents or Guardians will need to complete a health management plan for these students upon enrollment. Please contact Mrs. Hortman for information about these plans.

No medications will be administered to students under the age of 18 without parental consent.  All medication (prescription and non-prescription) should be brought to the clinic.  Emergency medication that is kept with a student should be documented on a form in the clinic.
