In order to complete a withdrawal form for Phoenix High School, you will need to follow these steps:
- Click on the Phoenix High School Withdrawal Form and download the form to your device.
- Open the downloaded form with an Adobe application.
- Fill out the form in Adobe.
- Click on the submit button at the bottom of the form. An email will be generated. Attach the completed withdrawal form and a picture of the signing parent's picture ID to the email.
- Click send to complete the process.
